Southeastern Regional Transitional Treatment Home
The Ministry of Community and Social Services' (MCSS) strategy to build upon existing community strengths, and address current gaps in specialized resources, has been making its way to reality with the creation of four treatment beds for the Southeast region of Ontario.

The creation of ninety specialized accommodation spaces across the province over the next two years was announced as part of the Ministry's transformation of developmental services in May, 2005. Four of those beds were allocated to the Southeast region. In the Fall of 2006, MCSS revealed operational and capital funding for the treatment beds with the objective of providing support to adults with a dual diagnosis (developmental disability and mental health issues and/or challenging behaviour).

An initial proposal and conceptual framework for the treatment home was developed collaboratively by Developmental Services of Leeds and Grenville, Pathways to Independence and Ongwanada. The proposal was submitted to the Southeast Regional Office of MCSS and approved with Ongwanada identified as the service provider.

This four-bed home will provide transitional treatment to support adults from across the Southeast Region who have a dual diagnosis (developmental disability and mental health issues and or challenging behaviour). Ongwanada has been collaborating with community partners across the Southeast Region in establishing this home which will be completed this summer.
 
Service Model

  • Transitional residential treatment
  • Treatment period: 6 months - 2 years
  • Biopsychosocial approach
  • Multi-disciplinary team
  • Treatment supports include
    • 24 hour residential support
    • Training in activities of daily living
    • Assessments
    • Specialized clinical intervention
    • Development and implementation of an individualized support/treatment plan
    • Stabilization
    • Caregiver training
    • Transition/discharge planning
 
Eligibility
  • Adults 18 years of age or older
  • Must have a confirmed diagnosis of developmental disability and mental health issues or challenging behaviour
  • Must live in one of the following counties within the Southeastern region
    • Lanark
    • Leeds and Grenville
    • Frontenac, Lennox and Addington
    • Hastings/Prince Edward
 
Admission Criteria
Individuals referred to the treatment home must have:
  • An ongoing commitment from their lead agency/caregivers to participate in the treatment process and to implement team recommendations upon discharge
  • Reserved funding with agency and family commitment for the client to return home
  • Be medically stable
  • Have a case manager
