POSITION:
To oversee and coordinate optimum health and wellness of the persons served at the Transitional Age Youth Facility supported by Ongwanada. To support the interdisciplinary team in the development of policies and procedures related to health and wellbeing and to make recommendations for and support the development of health related education. To also act as a liaison between the physician, outside agencies, person supported, family and the rest of the interdisciplinary team, in addressing health care needs.
DUTIES:
• Supports in the development of policies and procedures specific to the supported population
• Organizes and coordinates supported individuals health related admissions and discharges
• Develops resources specific to the supported population to aid in accessing community health supports
• Helps to facilitate engagement of supported individual, family and unregulated care providers through health education and teaching specific to the environment
• Works in a manner that incorporates health promotion, harm reduction, trauma-informed care, cultural sensitivity and anti-stigma approaches; recognizes the social determinants of health

QUALIFICATIONS, SKILLS & KNOWLEDGE REQUIRED:
• Current unrestricted registration as a Registered Nurse in the province of Ontario;
• Bachelor’s degree required
• Minimum five years of related experience in providing support to youth and young adults with mental illness, addictions, responsive behaviours and developmental disabilities
• Training in ASIST, anti-human trafficking, overdose prevention and trauma informed care is an asset
• Incumbent must have current CPR (BCLS) complete re-certification as per Ongwanada standards;
• Must successfully complete and maintain NVCI Training as per Ongwanada policies;
• Must possess a valid G Driving License, full driving privileges, insurances and access to own vehicle;
• Experience in health teaching and presentation skills (i.e. in services, department meetings);
• Excellent written and oral skills (i.e. policy writing, proposals), Effective interpersonal skills;
• Acquired knowledge relevant to the nursing profession and the application to improve the quality of his/her professional knowledge;
• Incumbents must be able to work independently with minimal supervision.
