COVID-19 vaccination clinic for people with developmental disabilities

Ongwanada, in partnership with Kingston, Frontenac, Lennox, and Addington Public Health, will open the COVID-19 vaccination clinic at Ongwanada (191 Portsmouth Ave.) beginning the week of April 26 to adults in the region (Kingston, Frontenac, Lennox, and Addington) with developmental disabilities that have yet to receive their vaccine. The clinics are scheduled to run on Tuesdays and Thursdays. One caregiver will also be allowed to book an appointment to receive their COVID-19 vaccination.

When asked in the drop-down menu for eligibility reason, select:

congregate living or highest risk health condition 

 

What should I bring to the appointment?

-Health card

-Completed consent form

-Documentation/confirmation from DSO or ODSP
