Community Network of Specialized Care

Ongwanada serves as the accountable agency for the Community Network of Specialized Care (CNSC) in the South East region. The CNSC is responsible for the provision of complex support coordination, dual diagnosis justice case management and health care facilitation for adults with developmental disabilities who have high support and complex care needs.

The CNSC team works with individuals, families and caregivers to facilitate and coordinate planning and support to enhance existing resources and services for those with complex needs and who require services from multiple sectors. The CNSC liaises with different sector partners, including the criminal justice system and health care system to assist with the navigation of these systems. The CNSC also assists with referrals and facilitating access to and from specialized resources at the local, regional, and provincial levels.

What does CNSC do?

Community Network of Specialized Care (CNSC) provides direct complex support coordination to adults with developmental disabilities with high support and complex care needs, or who require appropriate diversion from the justice system. The CNSC coordinates support and service within and across sectors by providing complex support coordination for people; acting as a direct resource to service agencies, Developmental Services Ontario, and local planning tables (including urgent response and service solutions/case resolution).

How do I access help from CNSC?

Individuals meeting eligibility criteria may access CNSC services through a referral from Developmental Services Ontario. Please connect with them and they will be able to assist you, the main contact number is 1-855-277-2121. Web: www.dsontario.ca

Is there a special CNSC agency for my area?

Eight agencies across the province are serving as accountable agencies to the Ministry of Children, Community and Social Services (MCCSS) for funding, coordinating and overseeing the delivery of the four functions under the CNSC mandate. Ongwanada is the accountable agency for the South East region which covers the areas of Kingston, Frontenac, Lennox and Addington, Hastings-Prince Edward, Lanark, Leeds and Grenville.
