Ongwanada, in partnership with Kingston, Frontenac, Lennox, and Addington Public Health, will open another round of vaccination clinics at Ongwanada (191 Portsmouth Ave.) on Thursday, May 20, Tuesday, May 25 and Thursday, May 27, to adults in the region (Kingston, Frontenac, Lennox, and Addington) with developmental disabilities that have yet to receive their vaccine. One caregiver will also be allowed to book an appointment to receive their COVID-19 vaccination. More appointments could become available so, continue to monitor the link for availability.

-Health card

-Completed consent form

-Documentation/confirmation from DSO or ODSP

 

Print and complete this COVID-19_vaccine_screening_consent_form for each individual receiving a vaccination.
